You need: 1) Active MC Authority certificate, 2) Certificate of Insurance ($1M Auto Liability / $100K Cargo), 3) Signed W-9 form with IRS EIN, 4) Driver CDL & Truck Cab Card, and 5) Notice of Assignment (NOA) from your factoring company if applicable. You can start within 24 hours.
